You should download the library from the server.They were named csv and inv.The path is under the $AU_TOP/plsql
And setup your regedit report60_path like you setup form60_path.

Did you check your customize program library?Because I open the file that you upload ,I did not see any Attached library.
The original program should have the Attached library.
